Baptist Health Internet Privacy Policy

This document outlines the policies and practices of Baptist Health System, Inc., a Florida not for profit corporation d/b/a Baptist Health ("Baptist_Health"), regarding the collection and use of personal information about employee, visitor, patient and general public users (collectively, "Users") of the various web sites and web pages operated and maintained by Baptist Health and its affiliates (collectively, the "Baptist Health Web Site").

To effectively monitor usage and improve the quality of the Baptist Health Web Site, Baptist Health reserves the right to gather general information about the use of the Baptist Health Web Site. Baptist Health may utilize server logs and industry-standard web site statistics software to count the number of Users visiting the Baptist Health Web Site and to count the number of views of particular pages, as well as to track other general usage statistics. As a general practice, Baptist Health will not track specific lawful Users' visits to the Baptist Health Web Site. At times, Baptist Health may find it necessary or desirable to share with third-parties certain aggregate, anonymous information about overall usage of the Baptist Health Web Site, in order to provide general information about our Internet services, sell advertising or establish sponsorships, or other business relationships. Baptist Health System Foundation, Inc., a Florida not for profit corporation d/b/a Baptist Health Foundation (the "Foundation"), 841 Prudential Drive, Suite 1300, Jacksonville, Florida 32207, 904.202.2919, is a 501(c)(3) tax exempt organization that performs charitable fundraising on behalf of Baptist Medical Center, 800 Prudential Drive, Jacksonville, Florida 32207; Baptist Medical Center Beaches, 1350 13th Avenue South, Jacksonville Beach, Florida 32250; Baptist Medical Center Nassau, 1250 South 18th Street, Fernandina Beach, Florida 32034; Baptist Medical Center South, 14550 St. Augustine Road, Jacksonville, Florida 32258; and Wolfson Children's Hospital, 800 Prudential Drive, Jacksonville, Florida 32207. Donations to the Foundation are very much appreciated, and the Foundation acknowledges all gifts on the above-referenced hospitals' behalf.

According to IRS guidelines, the amount of any deductible contribution to the Foundation for federal income tax purposes is generally the excess of the amount given over the value of any goods or services provided to the giver by the Foundation. Online donations to the Foundation should be tax deductible to the fullest extent permitted by applicable law, as no goods or services are provided in consideration of such gifts. Givers are encouraged, however, to consult with their tax advisors as to the deductibility of any donation to the Foundation.

A copy of the official charitable organization registration and financial information for the Foundation may be obtained from the State of Florida Division of Consumer Services by calling toll-free within Florida 1-800-HELP FLA (1-800-435-7352). Registration does not imply endorsement, approval or recommendation by the State of Florida. Florida registration no. CH3625. While the Foundation welcomes contributions from generous individuals and entities domiciled outside of the State of Florida, the Foundation does not actively solicit contributions outside of the State of Florida, either via an interactive web site or via direct or electronic mailings promoting the Foundation or the Foundation's page(s) on the Baptist Health Site.

The Foundation accepts online donations via an encrypted web form. As part of any such online donation, the Foundation collects standard credit card information (i.e., name, card number, card type and expiration date) and keeps a record of the financial transaction in a secured database. Credit card information collected will be used solely to process the donation. While the Foundation strives to maintain physical, electronic and administrative safeguards to (i) prevent unauthorized access to information collected, (ii) maintain data security and (iii) ensure the correct use of information collected (for example, the Foundation collects such information and processes such financial transactions via secure socket layer (SSL) encrypted sessions, which conform to the industry standard for securing web-based transactions), the Foundation cannot guarantee and does not represent or warrant that the use of its online donation form is invulnerable to security breaks and that information submitted thereby is always secure.
